29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system: a) Jobsite: On March 21, 2019, and at times prior thereto an employee performed roofing activities on an 8 in 12 pitched roof. Fall protection was not used. This exposed the employee to fall hazards ranging from 15 to 23 feet to the ground below. Note) Certification of corrective action is required for this item.

The employee did not recognize or acknowledge the hazards of working without a fall protection system, nor did the employee take action to minimize or correct the hazard.
