On or about March 11, 2019, located at Stadden's Bridge in Newark, Ohio, employees operated a Komatsu hydraulic excavator model PC228USLC-10 with a fork attachment that was fabricated in-house that did not comply with the Power Crane and Shove Association Standards number 3 for Mobile Hydraulic Excavator Standards for having a capacity rating available to the operator for the forks.
