29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:   Bowling Alley:  Employees performing maintenance and repairs on pin setting machinery were exposed to hazards associated with the accidental release of stored energy types such as electrical and mechanical (belts and pulleys, pin setter).  Although a process is followed, procedures are not in writing and do not encompass the requirements of this section.

29 CFR 1910.132(d)(1): The employer did not assess the workplace determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE):    Bowling Alley: The employer did not assess the workplace to determine if employees, particularly maintenance workers, should be required to wear personal protective equipment such as ear plugs or muffs to protect against excessive noise, and eye protection when performing mechanical maintenance duties or grinding metal pieces, and gloves when using chemical products for janitorial services.  The assessment needs to be written,  signed, and dated by the assessor.
