29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: A) 36 Mascolo Road building: On or about February 12, 2020, a coil machine was found to be lacking proper guarding in two areas, first at the point of operation where bending and sheering occur and second, in the area where the spindles are located exposing employees to amputation and crush hazards. B) 36 Mascolo Road, middle of shop area: On or about February 12, 2020, A large ProFab hydraulic bender was found to be lacking proper guarding exposing employees to amputation and crush injury hazards.

29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposed an employee to injury, was not guarded: A) 36 Mascolo Road, front of shop area: On or about February 12, 2020, a portable cam actuated press (button machine) was found to be lacking proper guarding, exposing employees to punctures, burns, and crushing injuries. B) Building at 36 Mascolo Road: On or about February 12, 2020, the ProFab three foot hydraulic bender was found to be lacking any type of guarding, exposing employees to amputations and crush injuries.

29 CFR 1910.255(b)(4): All press welding machine operations, where there was a possibility of the operator's fingers being under the point of operation, were not effectively guarded by use of a device such as an electronic eye safety circuit, two hand control: 36 Mascolo Road building: Near the front of the shop: On or about February 12, 2020, an employee was observed using the pinspotter resistance welder with the foot pedal from the side of the machine, freeing his hands to be exposed to a potential penetrating injury from the pinspotter.

29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition(s): 49 Mascolo Road, shop area: a Caterpillar forklift was found to be emitting carbon monoxide at 273 ppm when a four gas monitor was held approximately three to four feet away from the exhaust while it was running, exposing employees to potential carbon monoxide poisoning.
