Hazardous chemicals used in the facility included but were not limited to Methylene Chloride.

Abatement Certification and Documentation is required COMPANY-WIDE: On or about and at times prior to January 2, 2020, the employer did not accurately complete OSHA Form 300 in that for the calendar year 2019 in that: a) On or about December 31, 2019, an employee sustained an injury to her thumb which resulted in restricted duty. The injury would be appropriately captured with a check in Column I (Restricted Duty/Transfer), 2 days restriction in Column L, and a check in Column M1.
