On or about March 28, 2019, at I Supply Co., Fairborn, OH, an OSHA Form 301 or equivalent for the following work-related injuries or illnesses was not filled out completely or correctly for the calendar year 2019.    (a)  On or about January 16, 2019, an employee suffered a lower back injury while bending over to pick up product.  The employer did not complete the One Source Initial Report on Work Related Injury or Illness (equivalent form to OSHA 301) with the weight of the product lifted, the location of the task, a description of what happened and the signature of a supervisor with the date.    (b)  On or about February 10, 2019, an employee suffered abdominal pain and lumbar strain while lifting boxes in the warehouse freezer.  The employer did not complete the One Source Initial Report on Work Related Injury or Illness (equivalent form to OSHA 301) with the weight of the product lifted, a description of what happened and the signature of the employee with the date.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET)
