Sunset Suite D, Longmont, CO: On and preceding March 26, 2019, the employer did not establish clear and specific procedures to outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy in that the instructions concerning energy control were general. Employees from maintenance and production were routinely exposed to the hazards of inadequate lockout procedures during maintenance and cleaning activities for, but not limited to, equipment such as the HASL, scrubbers, driller/routers, presses, and ovens. This condition exposed employees to hazards associated with equipment or machines that contain multiple sources of energy capable of injuring an employee while performing servicing, cleaning, and maintenance activities.

Sunset Suite D Longmont, CO: On and preceding March, 27, 2019, the employer did not ensure where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use. Employees from the plating room were routinely exposed to corrosive chemical hazards during production or maintenance activities. Employees used chemicals such as but not limited to sulfuric acid 10%~1%, ss200 tin strip, promoter 3308. The eye wash and shower on the south side was blocked and was not inspected for suitable working condition in that when demonstrated, rust came out of shower head. This condition exposed the employees to corrosive chemical hazards.    (b) Circuits West, Incorporated, at 410 S. Sunset Suite D Longmont, CO: On and preceding March, 27, 2019, the employer did not ensure where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes were not provided within the work area for immediate emergency use. Employees from the plating room were routinely exposed to corrosive chemical hazards during production or maintenance activities. Sunset Suite D Longmont, CO: On and preceding March, 26, 2019, the employer did not ensure that machine guarding was provided to protect operator(s) and other employees from hazard(s) such as those created by ingoing nip points and rotating parts. Operators were routinely exposed to safety hazards during production on the A5 Neo Flying Probe machine because the protective hood was not closed. The machine was set to operate with the keyed service switch in a position which defeated the interlock to the hood guard. This condition exposed the employees to caught-in-between, crushing and amputation hazards.     (b) Circuits West, Incorporated, at 410 S. Sunset Suite D Longmont, CO: On and preceding March, 26, 2019, the employer did not ensure that machine guarding was provided to protect operator(s) and other employees from hazard(s) such as those created by ingoing nip points and rotating parts. Operators were routinely exposed to safety hazards during production on the Microcraft ELX 6146 machine because the front door guards were open. The machine was operating with the front door guards open because the safety interlock was defeated/bypassed. This condition exposed the employees to caught-in-between, crushing and amputation hazards.      (c)   Circuits West, Incorporated, at 410 S. Sunset Suite D, Longmont, CO: On and preceding March, 26, 2019, the employer did not ensure that machine guarding was provided to protect operator(s) and other employees from hazard(s) such as those created by ingoing nip points and rotating parts. Operators were routinely exposed to safety hazards during production on the Apollon-D1-F10 machine because the side cover had been removed for servicing and left in that condition for approximately one month. This condition allowed operators access to moving mechanical parts during production tasks and exposed them to caught-in-between, crushing and amputation hazards.
