Construction LLC on or about March 27, 2019 at the address of 102 Forest Gate Lot #13, Perrysburg, OH: The employer did not ensure employees were protected from falls while working from a job made scaffold platform. The employee was exposed to a 11ft fall to the level below while installing fascia to the residential structure. B) M.J.L. Construction LLC on or about March 27, 2019 at the address of 102 Forest Gate Lot #13, Perrysburg, OH: The employer did not ensure employees were protected from falls while working from a rough terrain fork lift platform (pallet) platform. The employee was exposed to a 18ft fall to the level below while installing fascia to the residential structure.
