29 CFR 1910.147(c)(4)(ii): The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, including, but not limited to Items of this section: 21 Third St, Palmer, MA On March 28, 2019 and at times prior, the employer's energy control procedures for log slitter/bologna cutter and rewinder machines did not list any pneumatic sources of energy powering machine components, specific devices to use to isolate pneumatic sources, or specific requirements for testing the machines to verify the effectiveness of energy control measures.

29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed: 21 Third St, Palmer, MA On March 28, 2019 and for seventeen months prior, the employer had not performed a periodic inspection that ensured each procedure was inspected for inaccuracies during an actual energy control task. The employer did not ensure that the inspector reviewed each authorized employee's responsibilities under each energy control procedure. The last inspection on October 23, 2017 was performed on an outside contractor who was not named on the form. Neither of the employer's two authorized employees had been inspected since February 11, 2007.
