29 CFR  1910.147(c)(4)(ii)(B): The energy control procedures did not clearly and specifically outline the steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy.  a. One Capital Drive, Suite 1, Cranbury, NJ - Slitter lines  The employer did not ensure that specific procedures were developed and utilized for the control of hazardous energy.  Employees engaged in the maintenance of equipment that included, but was not limited to, Arrow Converting Corp., Model 325 Slitter, performed lockout tagout procedures during the changing of the cutting blades on the equipment and were not provided specific procedures to control the release of electrical and/or mechanical energy.  Violation occurred on or about April 21, 2022

One Capital Drive, Suite#1, Cranbury, NJ - Slitter Line  The employer did not ensure that Authorized employees received hazardous energy control training. Employees engaged in the manufacture of tape products operated equipment that included, but was not limited to Arrow Converting Corp., Model 325 Slitter, where regular changes of the cutting blades were required, and the employee was not provided Lockout/Tagout training that included methods and means needed to isolate electrical and mechanical energy.  Violation occurred on or about April 21, 2022

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a. 1 Capital Drive, Suite#1, Cranbury, NJ - Drill press - machine shop  The employer did not ensure that machines that create hazards created by point of operation and rotating parts were guarded in a manner to prevent those hazards. Employees used a equipment that included, but was not limited to MSC Industrial Supply drill press where a guard was not installed to protect employees from rotating parts.  Violation observed on or about April 21, 2022  b. 1 Capital Drive, Suite#1, Cranbury, NJ - Lathe - machine shop  The employer did not ensure that machines that create hazards created by rotating parts were guarded in a manner to prevent those hazards. Employees engaged in the cutting of metal parts used Metal Lathe machines where the guard for the spindle and chuck did not adequately guard to prevent an employee from placing any body part in or near the rotating head.  Violation observed on or about April 21, 2022

29 CFR  1910.213(c)(1): Circular handfed ripsaw(s) were not guarded by an automatically adjusting hood which completely enclosed that portion of the saw above the table and above the material being cut:  a. One Capital Drive, Suite 1, Cranbury, NJ - Workshop area   The employer did not ensure ripsaws were guarded by an adjusting hood that enclosed the blade during operation.  Employees used a Delta Table Saw  for ripping of 2X4 wood studs with the blade extended approximately 3 inches with the automatic adjusting hood removed.  Violation occurred on or about April 21, 2022

Employees engaged in the production plastic film products operated equipment that included, but was not limited to Arrow Converting Corp., Model 325 Slitter (S-2) machines, which had chains and sprockets which were not completely enclosed to prevent employees from placing any body part or from reaching into the moving parts during production.
