29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2m) or more above a lower level was protected from falling by guardrail systems, safety net system, or personal fall arrest system: The employer is failing to protect employees from fall hazards associated with grain elevator activities. This was most recently documented on February 25, 2019, at the establishment located at 200 S. Main St, Cimarron, Kansas. The employer did not ensure that employees on the walking-working surface of the exterior west catwalk access for the flat storage structure were protected from falling by guardrail systems, safety net system, or personal fall arrest system. Employees engaged in grain handling activities in the flat storage structure were exposed fall hazards of approximately 16 feet from the unprotected side of the west catwalk access door platform to the ground below.

This was most recently documented on February 25, 2019 at the establishment located at 200 S. Main St., Cimarron, Kansas. Employees engaged in housekeeping and maintenance tasks were exposed to amputation hazards in that the v belts for the Westinghouse motor, model number ABFC, Serial number 6510 located in the basement of the distribution area were not fully enclosed. There was a 3 inch opening between guard and the frame.
