29 CFR 1926.20(b)(2): Such programs did not provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers:    At 3032 Chippewa Street, St. Louis, Missouri, the employer did not designate a competent person to inspect the job site for caught-in or struck-by hazards. One employee was observed working inside of an excavation positioned along the west side of the street without wearing a hard hat.  This same employee was observed working inside of a second, unprotected excavation located at the east side of the street without safe access or egress being provided. Loose, cobblestone bricks lined the top of the first excavation. The second excavation had a depth of approximately six feet.  This violation was observed on 04/02/19.

At 3032 Chippewa Street, St. Louis, Missouri, one exposed employee was working inside of an unprotected excavation.  The measurements of this excavation were approximately 6'D X 6'W X 7'L.  Neither a ladder or the equivalent were in use.  This violation was observed on 04/02/19.
