29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On or about April 2, 2019, on the West side of the building located at 2700 West Broward Blvd, Fort Lauderdale, Florida 33312, employees were placing formwork from a platform with an unprotected edge, exposing them to a fall of approximately 12'8".

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: On or about April 2, 2019, on the West side of the building located at 2700 West Broward Blvd, Fort Lauderdale, Florida 33312, an employee was using a ladder that did not extend three feet above the landing, exposing them to a fall of approximately 12'8".
