29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle.  (a) North, fabrication, AccurShear no. 637510 hydraulic shear: on or about March 13, 2019, the employer did not protect each employee in its regular production fabrication work tasks from the caught-in between hazards at the unguarded point of operation by the use of adequately adjusted guarding devices or other necessary means. Employees had their fingers exposed to caught in-between amputation hazards on the shear when hand-feeding and aligning metal parts in the unguarded point of operation for regular shearing fabrication tasks where the hold downs and the blade were accessible during normal operation.  There is no abatement certification or documentation required for this item.   (b) South, fabrication, Wysong hydraulic shear: on or about April 2, 2019, the employer did not protect each employee in its regular production fabrication work tasks from the caught-in between hazards at the unguarded point of operation by the use of adequately adjusted guarding devices or other necessary means. Employees had their fingers exposed to caught in-between amputation hazards on the shear when hand-feeding and aligning metal parts in the unguarded point of operation for regular shearing fabrication tasks where the hold downs and the blade were accessible during normal operation. 29 CFR 1910.213(c)(1): Circular handfed ripsaw(s) were not guarded by an automatically adjusting hood which completely enclosed that portion of the saw above the table and above the material being cut:    Southwest, wood shop, SawStop table saws: on or about April 2, 2019, employees were hand-feeding wood stock for ripping on the equipment with a missing point-of-operation blade guard. Employees had their fingers and hands exposed to contact with rotating blade.    There is no abatement certification or documentation required for this item.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:    North, fabrication, supervisor's desk, two relocatable power taps - on or about April 9, 2019, employees were exposed to fire and electrical hazards where the energized relocatable power taps were used in an industrial setting as substitutes for fixed wiring of electronic devices. Relocatable power taps was not directly connected to permanently installed branch-circuit receptacle outlets and were connected in-series to each other and powered by an extension cord. One relocatable power tap was affixed to a desk with a screw drilled through its enclosure. This equipment is not intended for such use.    There is no abatement certification or documentation required for this item.
