29 CFR 1910.147(c)(7)(i): The employer did not provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees:     Woodworking Dept., Schelling fh4 Cut-to-Size Saw #39.  On or about October 3, 2019 through March 11, 2020, machine operators who performed servicing and maintenance tasks such as blade changes and cleaning of saw dust in the saw blade area were exposed to amputation and crushing hazards to hands and fingers in the event of unexpected start up and release of stored energy of the rotating blades and blade motor carriage.  The employer did not ensure that lockout/tagout training, including a method for controlling movement of the blade carriage due to energy release, was provided to all employees who were assigned such tasks, to ensure that the purpose and function of the energy control program were understood by employees and that the knowledge and skills required for the safe application, usage and removal of the energy controls were acquired by employees.        Abatement documentation is required for this item in accordance with the requirements of 1903.19(d).

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):     Woodworking Dept., Schelling fh4 Cut-to-Size Saw #39.  On or about October 3, 2019 through March 11, 2020, machine operators who performed servicing and maintenance tasks such as saw blade changes and cleaning of saw dust inside the saw blade and blade motor area of the machine, were exposed to amputation and crushing hazards to hands and fingers from unexpected startup and/or release of stored energy.  Employees were instructed to use computer controls to over-ride a safety switch on the access door to the blade area as well as to activate an emergency stop button prior to entering the blade area danger zone and were not required to deenergize and lockout the machine's electrical and pneumatic energy sources.  Employees were not required to control hazardous energy sources in the following sequence:         1) turn off and shut down the machines,       2) physically locate and operate energy isolating devices in such a manner as to isolate the machines or equipment from the energy source(s),   3) affix lockout or tagout devices to each energy isolating device,       4) relieve, disconnect, restrain and otherwise render safe all hazardous residual or stored energy,     5) verify that isolation and de-energization of the machines or equipment have been accomplished.         Alternative protections, such as machine guarding, were not utilized.     Abatement documentation is required for this item in accordance with the requirements of 1903.19(d).
