29 CFR 1926.25(a): During the course of construction, alteration, or repairs, form and scrap lumber with protruding nails, and all other debris, was not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures:    a) Jobsite: On April 2, 2019 and at times prior thereto the controlling employer, A & A Construction & Development, Inc., did not ensure that their employees were protected and that the sub-contractor(s) protected its employees from puncture wound hazards where protruding nails were left in scrap wood.          Note) Certification of corrective action is required for this item.

29 CFR 1926.403(b)(1): Employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees:    a) Jobsite: On April 2, 2019 and times prior thereto the controlling employer, A & A Construction & Development, did not ensure their sub-contractors protected their employees from potential shock and fire hazards in where cardboard and other material was used as a cover for 120/208 volt power panels.      b) Jobsite: On April 2, 2019 and times prior thereto the controlling employer, A & A Construction & Development, did not ensure their sub-contractors protected their employees from the improper use of electrical equipment. A portable power distribution unit, "spider box", hanging by a power cord from a temporary power pole, was being used by a sub-contractor.      Note) Certification of corrective actions are required for these items.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws:    a) Jobsite: On April 2, 2019 and times prior thereto the controlling employer, A & A Construction & Development, did not ensure their sub-contractors protected their employees from the improper use of electrical equipment. A portable power distribution unit, "spider box", was hanging by a power cord from a temporary power pole.  Strain relief was not in place on both ends of the power cord.         Note) Corrected during inspection.
