29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s): (a) CDC Development Company dba Porchfront Homes at 2870 Duke Cir. Boulder, CO 80303: On or before 4/2/19 the controlling employer did not initiate or perform frequent and regular inspections of the jobsite, materials or equipment by a competent person. Subcontractor employees were working without fall protection on the roof and on an improperly erected pump jack scaffold. These conditions exposed subcontractor employees to fall hazards.

29 CFR 1926.25(a): During the course of construction, alteration, or repairs, debris was not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures: (a) CDC Development Company dba Porchfront Homes at 2870 Duke Cir. Boulder, CO 80303: On or before 4/2/19 the controlling employer did not ensure that work areas, walk ways and in and around the jobsite were kept clear of debris. The entire jobsite was littered with construction debris including nails, scrap wood and other material. This condition created slip, trip and fall hazards as well as puncture and laceration hazards.

Boulder, CO 80303: On or before 4/2/19 the controlling employer did not protect subcontractor employees from injury in that three basement window wells on a new single family home were not guarded and secured to prevent falls. These conditions exposed subcontractor employees to a fall hazard of seven feet.
