29 CFR 1910.212(a)(2): Guards were affixed to the machine and were operated in a way that they represent a hazard in itself. On or about February 15, 2019, employees were operating the B-4 machine (wire drawing machine) with the front guard lifting mechanism not operating properly. This condition exposed employees to struck-by hazards.
