On or about 4/03/2019, Nerry Humberto Tuj Mejia DBA Nerry Humberto Tuj Mejia was installing roofing materials on a roof of a residential building located at 104 Fox Grove Drive in OFallon, MO. The crew lead and five employees were not utilizing guardrail systems, a safety net system, or personal fall arrest systems while working near the edge of a 10/12 sloped roof approximately nine to twenty-seven feet above the ground.
