29 CFR 1910.147(d)(2): The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment:   A.  On or about April 4, 2019, Laser Room: An F&A Technician, Employee #1, making adjustments to the metal feeding device at the back of Automated Assembly Laser Machine #146 did not turn off or shut down the equipment using the company's machine-specific shutdown procedures.  The Equipment Operator, Employee #2, was not notified that work was being done on the equipment.  Employee #1 removed the safeguard from the side of the metal feeding device and while making adjustments to the unguarded parts, the equipment was activated.  Employee #1 suffered a severe laceration and broken bone in the left middle finger.
