Department 163, Revolver Cylinder Line:  On, or about,  April 4, 2019, side guards were not provided on the Nichols horizontal milling machine.     D.  Department 163, Revolver Cylinder Line, Daewoo Puma 6S:  On, or about, April 4, 2019, the spindle did not stop rotating when the access door was opened.    E.  Department 165, Slide Line:  On, or about, April 4, 2019, the door interlock mechanisms were bypassed on two Milltronics HM20 milling centers, #1 and #2.    F.  Department 353, Tool Room:  On, or about, April 25, 2019, a side guard was not provided on the Powermatic vertical belt sander, bt136390, for the upper pulley.      G.  Department 129, Lower Receiver Line, Old Broach, Op. 100:  On, or about, April 25, 2019, the two hand trips provided on the machine did not offer adequate safeguarding in that the operator could reach into the broach area during both of the passes of the tooling in the automatic mode.      H.  Department 150, Rifle Assembly, Government Line, Denison hydraulic press:  On, or about, May 17, 2019, the two hand trip buttons provided to cycle the Front Sight Taper Pin press were located within the calculated minimum safety distance.  The calculated safety distance was 28.9 inches from the die area and the actual distance was 9 inches.      I.  Department 150, Commercial Line, Denison hydraulic press:  On, or about, May 17, 2019, the two hand controls provided on the pin insertion press were not did not have an anti-tie down feature.
