29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about April 1, 2019, an employee was exposed to amputation hazards from unguarded moving parts/in-going nip points on AB 17, Angle Broom Machine, which resulted in an amputation from an in-going nip point between the arc and the fiber box. These unguarded moving parts/in-going nip points included the following: a. Between arc and fiber box, b. Fiber injection point at broom block, and c. Cam drives.
