An employee sustained an amputation of the tip of their left pinky finger when their finger was caught in the point of operation when attempting to unjam the plunger.

29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye: (a) Facility, 945 Vista Park Road, Pittsburgh, PA: On or about April 8, 2019 the employer failed to report an amputation that occurred on March 25, 2019. The employer became aware of the amputation on March 26, 2019 and failed to report the injury to the Pittsburgh OSHA Office within 24 hours. The employer reported the amputation on April 8, 2019.
