29 CFR 1910.147(c)(4)(ii): The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, including, but not limited to items of this section: (a) Facility, 945 Vista Park Drive, Pittsburgh, PA: On or about April 7, 2021, the employer did not have energy control procedures that clearly outlined the scope, purpose, rules and techniques to be utilized for control of hazardous energy. The employer's procedures only listed the equipment and energy source. There were no techniques listed that described how to control the energy (apply a lock, etc.), to test the equipment to ensure energy was controlled and/or procedures to take to return equipment to service.

29 CFR 1910.147(c)(7)(i):The employer did not provide adequate training to ensure that the purpose and function of the energy control program was understood by employees: (a) Facility, 945 Vista Park Drive, Pittsburgh, PA: On or about April 7, 2021 the employer had not provided training to persons who perform servicing of equipment. Employees who service equipment were shown the company's written program and told to read it and then sign off. The employer did not review the program or procedures with employees.

29 CFR 1910.212(a)(3)(ii):Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): (a) Facility, 945 Vista Park Drive, Pittsburgh, PA: On or about April 7, 2021 the opening between the guard and the table on the AlphaMachine underpinner was approximately 5 inches and the piece being pinned was 3/4 inch thereby not preventing potential contact with the point of operation. The employer, PrintDropper was previously cited for a violation of this occupational safety and health standard or its equivalent standard (name previously cited standard), which was contained in OSHA inspection number 1391641, citation number 1, item number 1, and was affirmed as a final order on May 28, 2019, with respect to a workplace located at 945 Vista Park Drive, Pittsburgh, PA.
