29 CFR 1910.95(c)(1): The employer did not administer a continuing, effective hearing conservation program as described in 29 CFR 1910.95(c) through (o) whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level of 85 decibels measured on the A scale, or equivalently a dose of fifty percent:  (a) At pallet repair station 1, a pallet repair operator was exposed to noise levels at 114.6% of the permissible 8-hour TWA (90 dBA).  The equivalent dBA level of 114.6% is approximately 90.9 dBA.  Sampling was performed for 392 minutes during one shift.  Zero exposure was assumed for the unsampled period of 88 minutes.  (b) At pallet repair station 2, a pallet repair operator was exposed to noise levels at 102.4% of the permissible 8-hour TWA (90 dBA).  The equivalent dBA level of 102.4% is approximately 90.1 dBA.  Sampling was performed for 361 minutes during one shift.  Zero exposure was assumed for the unsampled period of 119 minutes.  (c) At pallet repair station 3, a pallet repair operator was exposed to noise levels at 80.76% of the permissible 8-hour TWA (90 dBA).  The equivalent dBA level of 80.76% is approximately 88.4 dBA.  Sampling was performed for 342 minutes during one shift.  Zero exposure was assumed for the unsampled period of 138 minutes.  (d) At the saw shop, a trim saw operator was exposed to noise levels at 96.56% of the permissible 8-hour TWA (90 dBA).  The equivalent dBA level of 96.56% is approximately 89.7 dBA.  Sampling was performed for 426 minutes during one shift.  Zero exposure was assumed for the unsampled period of 54 minutes.

29 CFR 1910.133(a)(3): The employer did not ensure that each affected employee who wore prescription lenses while engaged in operations that involved eye hazards wore eye protection that incorporated the prescription in its design, or wore eye protection that could be worn over the prescription lenses without disturbing the proper position of the prescription lenses or the protective lenses:  (a) At pallet repair station 3, a pallet repair operator using a pneumatic nailer wore prescription eyewear and was not protected from eye injuries.

29 CFR 1910.242(b): Compressed air used for cleaning purposes was not reduced to less than 30 p.s.i.:  (a) In the pallet repair area, compressed air used for cleaning had a dead end pressure of approximately 100 psi.
