29 CFR 1926.20(b)(2): The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person:   On or about April 10, 2019 during the installation of roofing material at a new construction single family home addition located at 5486 River Summit, North Royalton, Ohio 44133; the employer did not conduct frequent and regular inspections of the job site where employees were exposed to fall hazards from working surfaces of heights approximately 12.3 feet above the ground.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about April 10, 2019, during the installation of roofing material at a job site located at 5486 River Summit, North Royalton, Ohio 44133; the employer did not ensure employees working on a roof of a single family home were protected from a fall of approximately 12.3 feet by utilizing a fall protection system.
