29 CFR 1926.102(a)(1): 29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles:   On or about September 24, 2019, at a worksite located at 10 and 11 Yesterday Lane and Lafayette Road, Medina, OH 44256, during residential roofing activities on a steep sloped roof, the employer did not ensure employees were protected from eye hazards by ensuring eye protection was available and worn while operating pneumatic nail guns at approximately 120 psi.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about September 24, 2019, at a worksite located at 10 and 11 Yesterday Lane and Lafayette Road, Medina, OH 44256, during residential roofing activities on a steep sloped roof, employees were exposed to a fall of approximately 12 feet. The employees were using an extension ladder to access the roof that did not extend at least three feet above the landing area.
