On or about April 9, 2019, at the above addressed jobsite, employees performing carpentry operations, installing floor joist, were exposed to falls of fourteen (14) feet created by the presence of unprotected sides.

On or about April 9, 2019, at the above addressed jobsite, employees working on top of the masonry wall installing floor joists were exposed to ends of rebar that were not guarded, thereby exposing employees to the hazards associated with impalement.

On or about April 9, 2019, at the above addressed jobsite, the job-made ladder used to access the second level did not extend at least three (3') feet above the upper landing surface, thereby exposing employees to the hazards associated with falls.    b.  On or about April 9, 2019, at the above addressed jobsite, the A frame portable ladder used to access the first floor did not extend at least three (3') feet above the upper landing surface, thereby exposing employees to the hazards associated with falls.
