29 CFR  1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Cross braces were used as a means of access.  a. On January 3, 2025, at the above addressed jobsite, an employee conducting carpentry operations from a welded frame metal scaffold was not provided with an access ladder, thereby exposing the employee to falls.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):  a. On or about January 3, 2025, at the above addressed jobsite, employees performing residential carpentry operations, thirteen (13') feet five (5") inches above the ground, were not protected from falls, thereby exposing employees to fall hazards.

29 CFR 1926.701(b): All protruding reinforcing steel, onto and into which employees could fall, shall be guarded to eliminate the hazard of impalement.  a. On or about January 3, 2025, at the above addressed jobsite, employees conducting carpentry operations working on the 2nd floor, were exposed to ends of rebar that were not guarded, thereby exposing employees to the hazards associated with impalement.
