29 CFR 1910.22(a)(1): The employer failed to provide a clean and sanitary condition for all places of employment, passageways, storerooms, service rooms, and walking-working surfaces. a) No-Bake Department - on or about April 11, 2019, debris, and empty 5 gallon containers were placed throughout the working area around the shaker, exposing employees to tripping hazards. b) 901 Front St. Whitehall, PA - on or about April 11, 2019, pneumatic air hoses were observed placed along walking areas when not in use, exposing employees to tripping hazards.

29 CFR 1910.28(b)(6)(i): Each employee less than 4 feet (1.2 m) above dangerous equipment was not protected from falling into or onto the dangerous equipment by a guardrail system or a travel restraint system, unless the equipment is covered or guarded to eliminate the hazard. a) No Bake Department - on or about April 11, 2019, employees pushed no-bake molds into the shaker which removed the sand, breaking the sand lumps into smaller diameter to be fed into the sand recycling system. The employee retrieved the mold from the shaker once completed. The employees performed this process less than four feet above the shaker and were not protected from falling in by a guardrail system or a travel restraint system.
