29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes: 13524 NW Industrial Dr., Bridgeton: Employer is exposing employees to hazardous conditions. Employees are permitted to work on roofs greater than 6 feet above ground level without using appropriate fall protection equipment where skylights are not guarded. On 04/10/2019 an employee who was working on a roof fell through an unguarded skylight. The employee fell approximately 18 feet onto a metal shelf. The metal shelf prevented the employee from falling directly onto a concrete floor.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: 13524 NW Industrial Dr., Bridgeton: Employer is exposing employees to hazardous conditions. Untrained employees are permitted to work on roofs greater than 6 feet above ground level without being provided fall protection training. On 04/10/2019 an employee fell through an unguarded skylight. The fall distance was measured at approximately 18 feet from the roof to a shelf. The shelf prevented the employee from falling onto a concrete floor. The employee was not trained to recognize fall hazards and the need for fall protection systems.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system: 13524 NW Industrial Dr., Bridgeton: Employer is exposing employees to hazardous conditions. The employees are permitted to work on roofs over 6 feet above ground level without using any forms of fall protection systems. On 04/10/2019 an employee who did not use any fall protection systems worked on a roof which was approximately 20 feet from ground level. The employer, R & A Contracting, was previously cited for a violation of this occupational safety and health standard or its equivalent standard CFR 1926. 501 (b) (10) which was contained in OSHA inspection number1229378, citation number 01, item number 01 and was affirmed as a final order on 06/05/2017, with respect to a workplace located at 21 Meramec Heights, Arnold, MO 63010.
