29 CFR 1910.177(c)(1)(ii): The training program for servicing rim wheels did not include, at a minimum,  the contents of 29 CFR 1910.177:  On or about April 10, 2019 employees were exposed to struck by hazards where the employer did not ensure that employees who operate the bead seating machines identified by the employees as the "blasters" on lines one, two, three and four had received the minimum required training to include the contents of OSHA's standard for servicing multi-piece and single piece rim wheels.

29 CFR 1910.177(d)(3)(ii): A rim wheel restraining device or barrier was not capable of preventing rim wheel components from being thrown outside or beyond the device or barrier for any rim wheel positioned within or behind the device:  a.  On or about April 10, 2019 employees were exposed to struck by hazards where the employer did not ensure that cylinder bolts on the line 2 bead seating machine were capable of preventing rim wheel components from being thrown outside or beyond the machine.    b.  On or about April 17, 2019 employees were exposed to struck by hazards where the employer did not ensure that cylinder bolts on the line 1 bead seating machine were capable of preventing rim wheel components from being thrown outside or beyond the machine.  c.  On or about April 17, 2019 employees were exposed to struck by hazards where the employer did not ensure that cylinder bolts on the replacement line 2 bead seating machine were capable of preventing rim wheel components from being thrown outside or beyond the machine.  d.  On or about April 17, 2019 employees were exposed to struck by hazards where the employer did not ensure that cylinder bolts on the line 3 bead seating machine were capable of preventing rim wheel components from being thrown outside or beyond the machine.  e.  On or about April 17, 2019 employees were exposed to struck by hazards where the employer did not ensure that cylinder bolts on the line 4 bead seating machine were capable of preventing rim wheel components from being thrown outside or beyond the machine.

29 CFR 1910.177(d)(3)(iii): A rim wheel restraining device or barrier was not visually inspected prior to each day's use and after any separation of rim wheel components or sudden release of contained air:  a.  On or about April 10, 2019 employees were exposed to struck by hazards where the employer did not ensure the weekly inspection cylinder bolts on the line one bead seating machine identified by the employees as the "blaster".  b.  On or about April 10, 2019 employees were exposed to struck by hazards where the employer did not ensure the weekly inspection cylinder bolts on the line two bead seating machine identified by the employees as the "blaster".  c.  On or about April 10, 2019 employees were exposed to struck by hazards where the employer did not ensure the weekly inspection cylinder bolts on the line three bead seating machine identified by the employees as the "blaster".  d.  On or about April 10, 2019 employees were exposed to struck by hazards where the employer did not ensure the weekly inspection cylinder bolts on the line four bead seating machine identified by the employees as the "blaster".
