Building 5223, supply technician office; on or about 16 April 2019 and times prior thereto; the periphery fan blades of a in wall mounted fan; make and model unknown; were not guarded with a protective barrier one-half inch or less. The measured distance from the floor to blade was 80 inches. This condition exposed employees to a caught in hazard.
