29 CFR 1926.1053(b)(13): The top or top step of a stepladder was used as a step: J Decker Homes project site located at Lot # 82 Creekside Drive, Waterloo, Illinois: On or about April 18, 2019, employees were exposed to fall hazards during construction of a one story 5/12 pitch residential home in that employees used the top rung of a six foot fiberglass ladder to gain access to an access hole to the roof.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): J Decker Homes project site located at Lot # 82 Creekside Drive, Waterloo, Illinois: On or about April 18, 2019, employees were exposed to fall hazards during construction of a one story 5/12 pitch residential home without fall protection while: a) Two employees were working on the roof, setting small truss sections and sheathing the roof; b) Three employees were working around and near an unguarded stairwell on main floor 10' above the ground floor slab; c) Three employees were working around and near four wall openings on back side main floor. The J Decker Homes was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1137169, citation number 1, item number 2 and was affirmed as a final order on May 2, 2016 with respect to a workplace located at 913 Stone Briar Drive, Fairview Heights , Il. 62208.
