29 CFR  1926.100(a): 29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:    On or about 10/13/2020, the employer did not ensure that head protective equipment was used when:  A.  Workers were exposed to impact hazards when they were handing roof decking sheets overhead to workers on the roof.    B.  Workers were exposed to impact hazards of falling material and/or tools when working in the fall zone of the roof.  The employer (John Decker II dba J. Decker Homes) was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1137169, citation number 1, item number 1 and was affirmed as a final order on 05/02/2016, with respect to a workplace located at New Construction Jobsite next to: 913 Stone Briar Drive, Fairview Heights, IL 62208.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:    On or about 10/13/2020, the employer did not protect employees who were engaged in residential roofing operations 6 feet (1.8 m) or more above lower levels.  The employees were conducting residential framing operations:  A.  Employees were exposed to the hazard of falling up to approximately ten (10) feet off of the truss plate when they were installing roof sheeting.    B.  Employees were exposed to the hazard of falling up to approximately nine (9) feet when walking across the trusses to get to the ladder.    C.  Employees were exposed to the hazard of falling up to approximately ten (10) feet out of the unguarded windows, at the rear of the structure.  D.  Employees were exposed to the hazard of falling up to approximately ten (10) feet off of the unguarded deck, at the rear of the structure.   E.  Employees were exposed to the hazard of falling up to approximately ten (10) feet through the unguarded basement stairway.  The employer (John Decker II dba J Decker Homes) was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1393454, citation number 2, item number 1 and was affirmed as a final order on 07/16/2019, with respect to a workplace located at Lot 82 Creekside Drive, Waterloo, IL 62298.  The employer (John Decker II dba J. Decker Homes) was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1137169, citation number 1, item number 2 and was affirmed as a final order on 05/02/2016, with respect to a workplace located at New Construction Jobsite next to: 913 Stone Briar Drive, Fairview Heights, IL 62208.

The employer (John Decker II dba J Decker Homes) was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1393454, citation number 1, item number 1 and was affirmed as a final order on 07/16/2019, with respect to a workplace located at Lot 82 Creekside Drive, Waterloo, IL 62298.
