29 CFR 1910.303(b)(1)(iv): Electric equipment was not free from recognized hazards that were likely to cause serious physical harm to employees, such as damaged electrical insulation:    On or about April 16, 2019, employees were exposed to electrical shock while using the # 3 spreader machine, and the employer did not ensure that the electrical insulation on the 12a 110v cord was examined.  The electrical insulation near the plug at the control panel was damaged and parts of the insulation had nicks and abrasions exposing the cords conductors.
