29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a. Crown Linen Service Inc., on the production floor, rotating shaft locate on the left side of the doffer roll on the feeder side of the Hypro II Flat Ironer # 3 was not guarded. b. Crown Linen Service Inc., on the production floor, rotating shaft locate on the right side of the doffer roll on the feeder side of the Hypro II Flat Ironer # 3 was not guarded. c. Crown Linen Service Inc., on the production floor, rotating shaft locate on the Left side of the doffer roll on the feeder side of the Hypro II Flat Ironer # 4 was not guarded. d. 29 CFR 1910.303(b)(1)(iv): Electric equipment was not free from recognized hazards that were likely to cause serious physical harm to employees, such as damaged electrical insulation: On or about April 16, 2019, employees were exposed to electrical shock while using the # 3 spreader machine, and the employer did not ensure that the electrical insulation on the 12a 110v cord was examined. The electrical insulation near the plug at the control panel was damaged and parts of the insulation had nicks and abrasions exposing the cords conductors.
