29 CFR 1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  a) Building Exterior-North Side: Employees were exposed to eye/face injuries while applying stucco from a pipe scaffold  without the use of eye/face protection. Employees were not protected with any type of eye/face protection, on or about 4/16/2019  b) Building Exterior-Northeast Corner: employees were exposed to eye/face injuries while mixing base coat without the use of eye/face protection. Employees were not protected with any type of eye/face protection, on or about 4/16/2019

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.   a) Building Exterior-North Side: A proper means of access was not provided for the scaffold system from the second tier up.
