29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence: On or about April 17, 2019, employees were exposed to machine hazards associated with moving parts and crushing when they engaged in change over activities on the Civiemmic Bundler. The employer did not ensure that energy control application steps to control the pneumatic and electrical energy sources were implemented prior to allowing employees to change the forks on the Civiemmic Bundler. The machine was not shut down or turned off prior to engaging in this servicing and maintenance activity. As a result, the remaining applicable energy control elements, involving machine isolation [1910.147(d)(3)], lockout/tagout device application [1910.147(d)(4)], dissipation of residual energy [1910.147(d)(5)(i)], and verification of isolation [1910.147(d)(6)] were not implemented to protect employees from machine servicing hazards."

29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc. (a) Civiemme Bundler (Mod. ST 100/A DX, N. Matr 012761011) located on Press 83 had unguarded in-running nip points where the "spaghetti" belt went around the roller exposing employees to caught in hazards. A guard had an opening of 7 1/8" width by 14 1/4" height allowing access. (b) Conveyor 4 located on the Press 83 had three unguarded in-running nip points where the belt went around the roller exposing employees to caught in hazards. (c) Conveyor 2 located on the Press 83 had two unguarded in-running nip points where the belt went around the roller exposing employees to caught in hazards.
