On or about April 16, 2019, at the above addressed jobsite, an inspection was not conducted by a competent person capable of correcting hazards such as falls, which could result in employee injuries.

On or about April 16, 2019, at the above addressed jobsite, employees performing residential roofing operations were exposed to falls of twelve (12) feet, thereby exposing employees to fall hazards.
