29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations present potential eye or face injury from physical, chemical, or radiation agents:  a. On or about July 29, 2020, at the above addressed jobsite, employees using pneumatic nail guns did not wear safety glasses, thereby exposing the employees to the hazards associated with eye injuries.

On or about July 29, 2020, at the above addressed jobsite, an employee was carrying material up an extension ladder, thereby exposing the employee to hazards associated with falls.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a.  On or about July 29, 2020, at the above addressed jobsite, employees performing residential roofing operations were exposed to falls of seventeen (17) feet above ground, thereby exposing employees to fall hazards.  b.  On or about July 29, 2020, at the above addressed jobsite, an employee performing residential roofing operations was exposed to falls of nine (9) feet five (5) inches above ground, thereby exposing the employee to fall hazards.  Yupa Construction Inc. was previously cited for a violation of this Occupational Safety and Health Standard 1926.501(b)(13) which was contained in OSHA inspection number 1432168, citation number 1 item number 1 and was affirmed as a final order December 12, 2019 with respect to a workplace located at 166 N. Vincent Drive, Bolingbrook, IL 60490.  Yupa Construction Inc. was previously cited for a violation of this Occupational Safety and Health Standard 1926.501(b)(13) which was contained in OSHA inspection number 1393628, citation number 1 item number 2 and was affirmed as a final order July 18, 2019 with respect to a workplace located at 202 Cedar Road, New Lenox, IL 60451.

29 CFR 1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer.  a.  On or about July 29, 2020, at the above addressed jobsite, employees had been provided fall protection training, however, the employer failed to keep a written certification of record.  Yupa Construction Inc. was previously cited for a violation of this Occupational Safety and Health Standard 1926.503(b)(1) which was contained in OSHA inspection number 1432168, citation number 1 item number 2 and was affirmed as a final order December 12, 2019 with respect to a workplace located at 166 N. Vincent Drive, Bolingbrook, IL 60490.  Yupa Construction Inc. was previously cited for a violation of this Occupational Safety and Health Standard 1926.503(b)(1) , which was contained in OSHA inspection number 1393628, citation number 2, item number 1 and was affirmed as a final order on July 18, 2019, with respect to a workplace located at 202 Cedar Road, New Lenox, IL 60451.
