29 CFR 1926.20(b)(2): Employer did not provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers.    a) Building 1118 Saber and Quill at Fort Knox Army Depot on or about 1/17/2019, the employer did not perform frequent and regular inspections of the job site to identify and correct hazards such as but not limited to site cleanliness, slips trips and falls hazards, struck by hazards, and electrical hazards.

29 CFR 1926.404(b)(1)(i): General. The employer did not use ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites. These requirements are in addition to any other requirements for equipment grounding conductors.    A) Building 1118 Saber and Quill on Fort Knox Army Depot, on or about 4/17/2019, the employer plugged equipment and various portable tools in to the building receptacles with out the added protection of GFCI's or an assured grounding program.

29 CFR 1926.404(b)(1)(iii)(C): Each cord set, attachment cap, plug and receptacle of cord sets, and any equipment connected by cord and plug, except cord sets and receptacles which are fixed and not exposed to damage, were not visually inspected before each day's use for external defects, such as deformed or missing pins or insulation damage, and for indications of possible internal damage. Equipment found damaged or defective shall not be used until repaired.      a) Building 1118 Saber and Quill on Fort Knox Army Depot: On or about 4/17/2019, the charging plug on the Sky Lift scissor lift was not grounded exposing worker to electric shock hazard.

29 CFR 1926.451(g)(4)(iv): When midrails are used, employer did not ensure chain was installed at a height approximately midway between the top edge of the guardrail system and the platform surface.  a) Building 1118 Saber and Quill on Fort Knox Army Depot, on or about 4/17/2019, the employer did not ensure the mid-rail height of the entry/ exit gate on the JLG Scissor lift 2630 ES was mid way between the top rail and the floor of the platform.
