OSH ACT of 1970 Section (5)(a)(1): : The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to fall hazards: On or about April 11, 2019, at a worksite located 1040 E. Walnut Street, Carbondale, Illinois 62901, employees were exposed to falling from a wooden trash basket that was elevated on the forks of a JLG Model 6042 forklift. The platform was not designed to safely elevate personnel, the platform was not secured to the forklift, and hoisted workers did not have control over the forklift.
