29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. Crossbraces were used as a means of access: On or about May 8, 2025, Buretta Construction & Management Inc. exposed employees to falling hazards while working from a Skyjack SJ6832RT, Serial number A200000988. The employer did not provide or require workers to use ladder(s) to access the upper working levels. Employees climbed up and onto the mid and top guardrails of the scissor lift at a worksite located at 2004 Saxony Lane in Jackson, Missouri.

29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems. On or about May 8, 2025, employees of Buretta Construction & Management Inc. were exposed to fall hazards when the employer did not ensure fall protection systems were utilized. Employees were installing structural roof supports between I-beams at heights ranging from 15-21 feet above the lower level at a worksite located at 2004 Saxony Lane in Jackson, Missouri.
