29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system.  A) Brandenberger Roofing L.L.C., on or about April 18, 2019 at the address of 5 Winfield Manor Court Lot# 2, Perrysburg, OH: The employer did not ensure employees were protected from falls while installing new roofing materials (asphalt shingles) to a new residential type structure.  The employees were exposed to a potential 10 and 20 foot falls to the level below.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards.  A) Brandenberger Roofing L.L.C. on or about April 18, 2019 at the address of 5 Winfield Manor Court Lot#2, Perrysburg, OH: The employer had not trained employees in the appropriate use of fall protection, such as but not limited to, the use of personal fall arrest systems while performing various roofing operations. The employees did not recognize the hazards associated with working from an elevated location (20 feet) without the benefit of fall protection.
