29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: On or about 4/17/2019, at a jobsite located at 1195 S. State Road 7, North Lauderdale, FL, the employer did not establish a LOTO program with regards to an employee exposed to electrical hazards when operating a JLG E300AJP boom lift to access a work area to place connection twist caps on exposed wires at a low voltage light fixture. PLEASE NOTE: CERTIFICATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.

29 CFR 1910.67(c)(2)(v): A personal fall arrest or travel restraint system that meets the requirements in subpart I of this part was not worn and attached to the boom or basket when working from an aerial lift: On or about 4/17/2019, at a jobsite located at 1195 S. State Road 7, North Lauderdale, FL, an employee was operating a JLG E300AJP boom lift to access a work area to place connection twist caps on exposed wires at a low voltage light fixture. The employee did not wear fall protection equipment and fell approximately 15ft - 20ft from the boom lift thus striking the ground. PLEASE NOTE: DOCUMENTATION AND CERTIFICATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION. TSF Lighting LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.453(b)(2)(v), which was contained in OSHA inspection number 1309711, citation number 01, item number 001, issued on 4/18/2018, with respect to a workplace located at 317 S. Cascade Drive, Springville, NY, which became a final order on 5/17/2018.
