29 CFR 1926.701(b): All protruding reinforcing steel, onto and/or into which employees could fall, was not guarded to eliminate the hazard of impalement:  a) Work site was located at 605 Pavonia Avenue, Jersey City, NJ 07306  Employees were exposed to impalement hazards as there was uncapped vertical rebar on the south side of the 15th floor where employees were working on the deck.  Violation was observed on or about April 17th, 2019.  b) Work site was located at 605 Pavonia Avenue, Jersey City, NJ 07306  Employees were exposed to impalement hazards as there was uncapped vertical rebar on the north side of the 8th floor where employees were storing their job boxes and using the porta-johns.  Violation was observed on or about April 17th, 2019.

29 CFR 1926.1153(f)(1): The employer allowed dry sweeping or dry brushing where such activity could contribute to employee exposure to respirable crystalline silica and wet sweeping, HEPA-filtered vacuuming or other methods were feasible:  a) Work site was located at 605 Pavonia Avenue, Jersey City, NJ 07306  A subcontractor's employees were exposed to health hazards as a worker on the 10th floor was drying sweeping construction debris and creating a cloud of dust that contained respirable silica without a respirator and without using water to suppress the dust.  Violation was observed on or about April 17th, 2019.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  a) Work site was located at 605 Pavonia Avenue, Jersey City, NJ 07306  A subcontractor's employee was exposed to fall hazards as he was working on the twelfth floor in close proximity to an unguarded leading edge exposing him to a fall of over one hundred feet to the ground below.  Violation was observed on or about April 17th, 2019.  AJD Construction Co., Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(1), which was contained in OSHA inspection number 1286022, citation number 1, item number 1 and was affirmed as a final order on 5/24/2019 with respect to a workplace located at 235 Grand St., Jersey City, NJ.  AJD Construction Co., Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(1), which was contained in OSHA inspection number 1103987, citation number 2, item number 1 and was affirmed as a final order on 9/6/2016 with respect to a workplace located at 615 Pavonia Ave., Jersey City, NJ.

Violation was observed on or about April 17th, 2019.  b) Work site was located at 605 Pavonia Avenue, Jersey City, NJ 07306  Employees were exposed to fall hazards as a guardrail section on the east side of the 15th floor consisted of a bent stanchion that was unserviceable.  Violation was observed on or about April 17th, 2019.  AJD Construction Co., Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.502(b)(3), which was contained in OSHA inspection number 1079403, citation number 1, item number 1 and was affirmed as a final order on 2/1/2016 with respect to a workplace located at 7 Navy Pier Court, Staten Island, NY.
