29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) On or about April 17, 2019, and at times prior, workers were exposed to caught-in hazards at a Dayton model 3NYA7combo belt and bench grinder where the belt pulleys guard on the left side was missing and exposing workers to ingoing nip points. Abatement Note: Abatement certification is required for this item (see enclosed Certification of Corrective Action Worksheet).

29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposed an employee to injury, was not guarded: a) On or about April 17, 2019, and at times prior, workers were exposed to caught-in hazards at a Clausing Colchester 15 horizontal lathe that was not fitted with a guard to cover the chuck area during operation. b) On or about April 17, 2019, and at times prior, workers were exposed to caught-in hazards at the Hartner vertical mill while the cutting tool is rotating with the operator is in close proximity to the unguarded point of operation. Abatement Note: Abatement certification is required for this item (see enclosed Certification of Corrective Action Worksheet).

29 CFR 1910.305(j)(4)(vi): An individual disconnecting means was not provided for each motor: a) On or about April 17, 2019, and at times prior, equipment wiring for the JMT PBF 2560 press brake did not include a disconnect switch between the equipment and the respective breakers in the panel board. b) On or about April 17, 2019, and at times prior, equipment wiring for the Haas TM 2P Vertical CNC mill did not include a disconnect switch between the equipment and the respective breakers in the panel board. NOTE: a single disconnecting means may be used for a group of motors under certain condition (such as several motors in a single machine as in item b above). Abatement Note: Abatement certification is required for this item (see enclosed Certification of Corrective Action Worksheet).
