29 CFR 1910.28(b)(1)(i): Except as provided elsewhere in this section 29 CFR 1910.28, the employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the systems described in 29 CFR 1910.28(b)(1)(i): (a) Ska Fabricating, Inc.: On or before 2/17/22 the employer did not protect employees from injury in that the MAD mezzanine did not have guard rails or other approved means of fall protection installed to protect employees from fall hazards. This condition exposed employees to an 8 foot fall hazard onto concrete. (b) Ska Fabricating, Inc.: On or before 2/17/22 the employer did not protect employees from injury in that the de-pal mezzanine did not have guard rails or other approved means of fall protection installed to protect employees from fall hazards. This condition exposed employees to an 7.5 foot fall hazard onto concrete. Abatement Note: Abatement certification is required for this item. 29 CFR 1910.178(a)(5): The powered industrial truck was equipped with front-end attachments other than factory installed attachments, however the employer did not request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered: (a) Ska Fabricating, Inc.: On or before 2/17/22 the employer did not protect employees from injury in that unapproved fork extensions were used on the Toyota sit down forklift to move large crates. The change in load center was not reflected in the data plate and the fork extensions had not been approved by Toyota. This condition exposed employees to caught-in, struck-by, and crushing hazards.
