29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    (a) Facility, 2319 Beans Cove Road, Clearville, PA:  On or about April 19, 2019 the employer did not develop written energy control procedures for all equipment serviced and maintained by employees.  Employees service equipment such as, but not limited to: debarker, hog, trimmer, edger and pellet machine.  The employer does not have any specific energy control procedures for any of the equipment to ensure employees are protected.

29 CFR 1910.147(c)(7)(i): The employer did not provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees:    (a) Facility, 2319 Beans Cove Road, Clearville, PA:  On or about April 19, 2019 the employer had not provided any training to employees who service or maintain equipment to ensure they understood the purpose and function of energy control or had the knowledge and skills for safe application usage and removal of control devices.  Employees service equipment such as, but not limited to: trimmers, edgers, debarkers, hog/chipper, or pelletizer and no training has been provided to ensure employees are protected when performing such tasks.

29 CFR 1910.265(c)(4)(iv): Elevated platform(s), 4 feet or more from the floor level, were not provided with a standard railing:    (a) Facility, 2319 Beans Cove Road, Clearville, PA:  On or about April 19, 2019 the elevated platform in front of the trimmer saw, measuring approximately 54 inches in height, was not provided with a standard railing.  Employees work from this platform to grade lumber.

29 CFR 1910.265(c)(5)(ii): Stairways were not provided with a standard handrail on at least one side or on any open side:    (a) Facility, 2319 Beans Cove Road, Clearville, PA:  On or about April 19, 2019 the 5 step stairway positioned at the grading platform did not have any handrails in place on either side for employees to use when traversing up or down.

29 CFR 1910.265(e)(4)(ii)(c): The rear of trimmer saw(s) was not guarded for the full width of the saw(s):    (a) Facility, 2319 Beans Cover Road, Clearville, PA:  On or about April 17, 2019 the rear of the trimmer saw was not guarded or protected to prevent employee contact with rotating parts.  An employee was positioned at the rear of the trimmer saw and opened a door to a sawing area.  The employee's sweatshirt caught on an unguarded sprocket and began wrapping around the axle.  The employee was caught in the equipment until it was powered off by another employee.  The injury to the employee resulted in the amputation of their left arm.

29 CFR 1910.265(c)(3)(iv): Floors, footwalks, and passageways in the work area around machines or other places where person(s) were required to stand or walk were not provided with effective means to minimize slipping and or tripping:  (a) Facility, 2319 Beans Cove Road, Clearville, PA:  On or about April 19, 2019 the inclined passageway between the operator's area of the trimmer saw and the grading deck and the platform area at the trimmer controls had sections of plywood installed that were not effective at preventing slipping or tripping hazards for employees who traveled through this area.
