Employee was observed installing wood materials using a nail gun without eye/face protection.

Employees were observed accessing and egressing the roof by way of a portable ladder that did not extend 3 feet past landing.  Employees were exposed to a fall distance of greater than 12 feet eave to ground without use of fall protection.
